Act No. 318 of 18 October 1999, "Ley para Reglamentar la Práctica de Hacer Tatuajes Permanentes en Puerto Rico", regulates permanent tattooing. In the consolidated version of the Office of Management and Budget (revised 4 November 2024), no one may tattoo without a licence and registration in the Department of Health's Register of Dermatograph Artists, and every studio must also be licensed. Article 9 prohibits tattooing people under 21.

On 7 March 2022, Nydia Bauzá (Primera Hora) reported that Governor Pedro Pierluisi had vetoed Senate Bill 286, which would have banned employment discrimination, public and private, against people with tattoos, piercings or hair dyed in unnatural colours. He cited an "error" (the same article of the Municipal Code amended twice in different ways) and "legal impediments", while writing that the measure "in principle has a very laudable intention". Senator María de Lourdes Santiago announced the bill would be refiled.
